If your district wants to purchase Student Accident coverage, you just need to complete the Enrollment Form and send it to American Fidelity General Agency with a check for the annual payment. These instructions are on the enrollment form. However, you may also submit your enrollment form and request that an invoice be mailed when your policy is issued.
Note: The enrollment form indicates that the deadline for enrollment is 8/1/2024, however, we can extend this deadline for any school that would like a later effective date for their student accident coverage. By participating in this program, your school district can provide peace of mind to students and their families, knowing that they are protected in the event of an accident. The Student Accident and Athletic Insurance program is tailored to meet the specific needs of Arkansas schools, and we believe it will be a valuable resource for your district.
